Has mexico or spain qualified for every single world cup?

And also I have another question. In the qualifications for 2010 Mexico beat USA 2-1 but I still saw USA play in the world cup. Then they played with Honduras and Honduras lost but Honduras got to play in the world cup. USA and Honduras were both looser countries in the qualifiers. Why did both get to play in the world cup? How do qualifiers work?
